FitKit Limited Guaranty

LIMITED GUARANTY FOR CUSTOM BRACES SIZED USING THE FIT KIT

Bledsoe hereby provides the following limited guaranty for custom braces manufactured by Bledsoe pursuant to the specifications provided by a medical professional properly utilizing the FitKit ("the FitKit Limited Guaranty"). If a custom brace manufactured by Bledsoe pursuant to the specifications provided by a medical professional properly utilizing the FitKit fails to fit the original user of the custom brace in a manner that is reasonably satisfactory to the medical professional, Bledsoe will adjust the brace or make a new brace for the original user at no additional charge. In order to be eligible for the FitKit Limited Guaranty, the following terms and conditions must be, or must have been, strictly complied with by the medical professional:

(a) The medical professional must have properly followed the instructions when utilizing the Fit Kit, including, without limitation, correctly recording and transmitting the measurements provided by the Fit Kit to Bledsoe.

(b) The medical professional must contact the Bledsoe Customer Care Department at the number set forth below within five (5) days of discovery of any problems with the fit of the brace. Customer Care will issue a Return Authorization number to return the Product to Bledsoe at Bledsoe's expense.

(c) The medical professional must make a cast mold of the original user's knee/leg and send it and the brace back to Bledsoe.

(d) Based upon the cast mold, Bledsoe will recheck all measurements provided to Bledsoe by the medical professional. In the event that Bledsoe determines, in its sole discretion, that the measurements provided by the medical professional were correct, Bledsoe will promptly either repair or replace the brace, at the sole discretion of Bledsoe.

(e) In the event that Bledsoe determines, in its sole discretion, that the terms of this FitKit Limited Guaranty were not strictly followed, including, without limitation, a determination that the medical professional did not correctly record and transmit the measurements provided by the Fit Kit to Bledsoe, Bledsoe shall promptly notify the medical professional of such determination and this FitKit Limited Guaranty shall not be applicable.

MAKING A WARRANTY CLAIM

In the event a customer wishes to have a Product repaired/replaced on a covered warranty issue, the customer should contact the Bledsoe Customer Care department at 1-(800)-526-3666 within 5 days of discovery of the problem related to the warranty issue. Customer Care will issue a Return Authorization ("RA") number to return the product to Bledsoe at Bledsoe expense. Upon evaluation by our Quality Assurance team and/or management, all used products covered by our warranty or the FitKit Limited Guaranty shall be repaired or replaced at the sole discretion of Bledsoe.

This warranty grants specific legal rights. Customers may also have other rights that vary from state to state.

60-Day Pain Relief Guaranty

Bledsoe guarantees that if the Thruster RLF OA Knee Brace is properly fitted by a qualified medical professional on patients diagnosed by a licensed physician with unicompartment osteoarthritis and it does not provide significant* or sufficient* relief of the patient's OA knee pain within the first 60 days of use, Bledsoe will offer a full refund to the ordering facility of the amount actually paid to Bledsoe for the Thruster RLF OA Brace.

* Bledsoe does not and cannot guarantee that all knee pain will be completely eliminated. Sufficient or significant relief of pain is defined as a reduction in pain level within the first 60 days that is sufficient to cause the patient to continue to use the brace as an adjunct, alternative, or additional means of pain control when used in conjunction with other methods of pain relief as prescribed by the treating physician based upon sound current medical practices. Refund of the actual price paid is contingent upon receipt of the returned brace to the Bledsoe Corporate headquarters.
